Family Sponsorship Form

  • This agreement is 12 months in length.
  • Benefits may be split between productions where there are 2 productions during the 12 month period.
  • In the event that Leongatha Lyric Theatre (LLT) are not able to deliver the full sponsorship package, this agreement will be extended a further period of time in which the sponsorship package is fully delivered. 
  • Upon accepting the Family / Community Sponsorship Package, an invoice will be generated using information provided above.
  • Direct Deposit or EFTPOS is preferred however if you would prefer to mail in payment via bank cheque, the address is: Leongatha Lyric Theatre, PO Box 236, Leongatha 3953. 
  • This agreement shall remain in place for the length of time noted above unless withdrawn by business in writing prior to end date.
